Tomorrow morning (Tuesday, March 27) the Registration Department will be rolling out a new Patient Registration and Reference Form.
While not directly affecting most employees, we want you to be aware of the changes and be familiar with the form.
When registering for services, the patient will receive this form to help direct him/her to the appropriate department. Using the form will help expedite the pre-registration and registration processes and decrease unnecessary paperwork. Below is an image of the front of the form, and the reverse is a map of the hospital’s first floor.
Registration will also no longer be placing patient ID bands on patients receiving services on the first floor. If an ID band is needed, each department will now be responsible for printing and placing the band on the patient.
